By participating in a 410and7 Volleyball Tournament, I hereby declare that I will abide by all outlined rules, regulations and policies set forth by the team at 410and7. I understand that participating in an athletic event such as this may provide an inherent danger or hazard, and I take full responsibility for my actions and physical condition.

I agree that the 410and7 team is not responsible for any potential harm that may occur and I relieve them of any liability, cost or expense that may occur while participating. In case of an emergency, I give my permission to receive emergency medical treatment as seen fit by a professional.

410and7 may take pictures and/or videos of participants at all current and future programs, activities, or special events. I am aware that pictures and content can appear in future promotional material, including newsletters, website updates, and more. I allow the team at 410and7 to use any photos of me as they see fit.
